New Care Models - Five Year Forward View

TheLMC iscontacting you to gather information on the local development of new care models.

The Five Year Forward View of October 2014 sets out several new care models which aim to ‘dissolve traditional boundaries’ between general practice and hospitals, health and social care and mental health services.The document proposes creating a number of major new care models, recognizing that “ England is too diverse for a single model of care to work everywhere.” The new care models outlined in the document (though short on detail and open to local interpretation) are:

The first two of these proposed models look to redefine the relationship between primary and secondary care and are of significant interest to all GPs and practices.

The intention is to pump-prime and fast track a cross section of new care models, identifying current exemplars, potential benefits, risks and transition costs.National and regional expertise and support will be harnessed to ‘implement care model change rapidly and at scale’.[How these proposals develop in practice is of course contingent on the election outcome and reception by Government.]

The Five Year Forward View Into Action: Planning for 2015/16 expands on these proposals by inviting local organisations wishing to become ‘first cohort’ or ‘vanguard’ sites to express their interest by 2 February to a new care models team.

Please let the LMC know if you are involved with or know in your area of any potential first cohort site bids (either for MCPs or PACs) so that we are fully aware of all developments locally, as well, of course, as being able to offer advice and assistance when required.
